I am a community health worker and harm reduction advocate with more than eight years of experience supporting people experiencing homelessness, substance-use challenges, and housing instability in San Francisco. I provide compassionate, trauma-informed case management, street outreach, crisis intervention, wellness checks, housing navigation, and connections to medical, behavioral health, and treatment services. My experience includes developing individualized service plans, facilitating health education and community workshops, coordinating resources across public agencies and community partners, and maintaining accurate client documentation in WelcomeMat and the ONE System. I bring a strong understanding of Housing First, harm reduction, racial equity, and the value of lived experience in program design. I also founded Smart Harm Reduction & Drug Safety, a peer-led initiative that integrates prevention, workforce development, digital access, and AI-powered harm reduction strategies. I am skilled at building trust with high-need communities, supporting service-delivery goals, tracking outcomes, and collaborating across systems to improve access to care and stability.
